Youth Behavioral Health Navigator of Southwest Washington Serving Clark, Skamania and Klickitat Counties

The Youth Navigator program is a behavioral health care model in which we set up MDT’s (multidisciplinary team meetings) to connect our youth and families to the care and resources they need. We are committed to partnering with the community of Southwest WA to navigate the behavioral health needs of our youth and families by establishing trust, building relationships, inspiring hope, and walking alongside youth and families during the process. Our program is in run under Carelon Behavioral Health and is designed to meet youth and families where they are at in their journey navigating behavioral health resources. Children, youth, and families are the center and priority of our work. We collaborate with community stakeholders to bring them together with the intent to wrap around the youth and family to ensure they receive the care, support, and services needed to address their complex behavioral health concerns. Our long-term vision is to foster community relationships that will partner with us to reduce the occurrences of emergency room and inpatient hospitalizations due to complex behavioral health concerns.

Program Update November 2025: The Southwest Washington Youth Navigator Program team received over 500 referrals for youth since the program launched in March of 2023. We proudly served each youth and family that has been referred to us and have worked to built lasting partnerships with our Southwest Community. We would like to thank our many community partners who have come together to meet with youth and families every week and have put countless hours into supporting our program!

Southwest Washington Family, Youth, System Partner Round Table (FYSPRT): The Southwest Washington Family, Youth, System Partner Round Table is a dedicated platform for families, youth and system partners to collaborate, listen and incorporate community voices into behavioral health service decision making in the Southwest region and the state level.
