Community Multidisciplinary Team
(MDT) Referrals
The Behavioral Health Navigator Community MDT is a community-based, family-focused multidisciplinary team of community stakeholders, providers, and advocates who aim to assist youth and their families with complex behavioral health presentations, care coordination and case planning. The MDT can assist with tasks such as: outpatient service recommendations, care coordination needs, behavioral management strategies, safety planning strategies, transitional planning, discharge planning and family engagement strategies for youth and families who reside in Clark, Skamania, and Klickitat counties.

If possible, we ask that you obtain an a signed release of information (ROI) (Español) from the legal guardian at the time of referral to our program as this helps us get started pulling resources together quickly for the youth and family. If you are not able to obtain an ROI you may still make a referral to our program and we will assist in obtaining an ROI. If you are a youth or family referring yourself or a family member we can also help walk you through the process of getting a signed ROI to us.
